THE FACING HISTORY SCHOOL IS NOW 6TH THROUGH 12TH GRADE!
has a School-Based Health Center (SBHC) run by Ryan Health. The SBHC is staffed by Ryan Health-licensed professionals, consisting of medical and mental health providers.
Please know that your child can use the SBHC and see your other doctors as well. Signing a consent form does not change your insurance, your private doctor, or affect the number of times your child can see their primary doctor.
At the SBHC, your child can receive the services listed at no cost to you, regardless of insurance status. Although the SBHC may bill insurance, there are no co-pays for you and you do not receive a bill.
School-Based Health Center Services include:
Complete physical examinations
Medications and prescriptions
Medical laboratory tests
Immunizations
Medical care and treatment for acute and chronic conditions
Age-appropriate reproductive health care
Health education and counseling
Mental health counseling and services
Short-term therapy
Group therapy
Family therapy
Crisis intervention
Screening for vision, hearing, asthma and obesity
Access to care 24 hours/day, 7 days/week
REGISTRATION AND CONSENT FORMS
Registration is required in order for your student to receive SBHC services. At the beginning of the school year, paper copies of the registration forms will be provided by the school. Parents/guardians can also complete the registration forms online via PandaDoc at the link below. If paper forms are completed, please return to the student’s homeroom teacher, school office or SBHC’s office.
